Out-of-State/International Applicants for Illinois License

1. Print FBI Fingerprint Cards

Print 2 copies of the FBI FD-258 fingerprint card form.
Use standard white printer/copy paper — printing on official cardstock is not required.
International Applicants: ➡️ Only use the FBI FD-258 form. Do not use non-FBI fingerprint cards.

2. Get Fingerprints Taken

Have 2 fingerprint sets taken using the ink and roll method.

Acceptable: Electronic fingerprinting printed onto FBI FD-258 by authorized providers (e.g., police department, fingerprint contractor).
Mark the reason for fingerprinting on the form. Include ORI Number:
Locate the ORI (Originating Agency Identifier) Number on the provided ORI Sheet.
Enter this number in the designated ORI field on the FD-258 form.

3. Complete the Identity Verification Certifying Statement (OOS-FP)

Section 1: Completed by you, the applicant.
Section 2: Completed by the law enforcement officer or fingerprint technician who takes your fingerprints.

4. Fill out the Out-of-State Fingerprinting/Consent Form

Include a phone number or email address for contact.
Sign the form to confirm you understand the data privacy and biometric policy.

5. Include Payment – $67.00

Preferred: Credit card (use the payment form).
Also accepted: Money Order.
NOT accepted: Cash or personal checks.

6. Double-Check Your Mailing Packet

Make sure the following items are included in your envelope:

✅ FBI FD-258 fingerprint card (completed)
✅ Out-of-State Fingerprinting/Consent Form (completed and signed)
✅ Identity Verification Certifying Statement (OOS-FP) (Sections 1 & 2 filled)
✅ Payment – Credit Card Form, or Money Order.
